The goal of Ludlowe Care Center’s Rehabilitation
department is to promote the highest level of
independent functioning for all residents and
patients. For long term residents, this objective
is complemented by additional therapies to prevent
avoidable decline in function and independence.
Physical, Occupational and Speech Therapy services
are available 7 days per week, as needed and prescribed
by an individual’s attending physician.
All therapy services are individually designed
based upon the needs and capabilities of the resident/patient
and are directed by experienced and licensed therapists.
National's "Passport Rehabilitation" program was specifically designed to meet the needs of individuals requiring a short-term rehabilitative stay following a hospitalization or an acute medical episode. While on your short-term "trip" with us, please take full advantage of the amenities and services our facilities offer. During your stay, if you find anything requiring our attention, please let us know as soon as possible so that we may make your stay the best it can be.
Social Services
Our Social Services department, staffed with concerned and
dedicated professionals, helps residents adjust to becoming
members of their new community. They work with the resident on
a one-to-one basis and serve as a liaison between staff and
families. They are here to answer questions and offer support
and guidance.
Short-term Recovery & Rehabilitation
Our rehabilitation care team is made up of experienced
occupational, speech, and physical therapists, as well as
nurses, social workers, and certified nursing assistants.
Their goals are the same as yours: focus on health and
wellness activities to maximize independence and minimize your
length of stay. We offer pre-discharge home evaluations from
our staff, patient and family education and home care services
planning, as well as resident and family services.

Recreation Programs
Our recreation program keeps residents socially involved with
the world around them. An important part of our care plan is
giving each resident an opportunity to participate in daily
activities. The recreation program at Ludlowe is diversified,
innovative, and carefully designed by a staff that truly cares
about the residents they serve. It is geared toward
independence and self-fulfillment. Volunteers are an integral
part of this team.
The recreation staff continually develops new programs based
on residents’ suggestions. Some of the many activities

Alzheimer’s/Dementia Special Care
This program is designed to meet the special needs
of Alzheimer’s/Dementia residents in a safe, caring
and secure environment. Our newly renovated and
spacious open floor design fosters a positive and
warm atmosphere, encouraging residents from all
walks of life tom participate in recreational
activities. Our goal is to help residents with
Alzheimer’s or related dementias maintain their
current level of functioning.
